PATIENT RESPONSIBILITY

 

  • Please arrive 15 to 30 minutes prior to scheduled visit.

 

  • Patients receiving allergy injections must wait in the office for a 20 to 30 minute observation period after every injection in case of an adverse reaction.

 

  • Have any paperwork completed prior to arrival, bring updated insurance information and a copay if you have one.

 

  • If you need to cancel or reschedule your appointment, please contact the office at least 24 hours prior to your appointment or you may be charged a deposit to reschedule. 

 

  • A parent or legal guardian must accompany children under age 18.  It is suggested that children of patients not attend the appointment due to the length of the appointment and COVID-19 precautions.

 

  • As a courtesy to other allergic patients, we ask that you refrain from wearing strong perfumes or colognes at the time of your visit.​

  • Please wear a mask while here in the office. 

​

  • Please reschedule if you are ill.
